Student Industrial Work Experience Scheme (S.I.W.E.S) Industrial Training (I.T) was designed by the government to add to student Curriculum Vitae (C.V) after graduation.
A successful SIWES should be able to land a student an A, which can boost Cumulative Grade Point Average (C.G.P.A). One of the things that can contribute to that is the proper filling of an IT log book.


The log book is a memo booklet given to all IT students before the commencement of the program. It is supposed to be carried along every day until the conclusion of the program. The logbook is divided into sections where student fills in their details and work experience.

The Student Page

The beginning of the book contains a page to be filled by the student. The information in it ranges from the name of the student, institution of study, the course of study etc. This is not the aspect we want to concentrate on. Even though there is no official score for filling this aspect very well, a student may fail if there is no information there or the information provided is wrong. However, it is practically a straight forward thing.

Log Page

These pages are also divided into daily activities for the next one week. At the end of the week, a separate page for summarizing all the activities for that week is provided. This method goes on for the next four week making it a month. A page is set aside for a summary of all activities of the previous month. This goes on for the six months period.

The daily activity page contains a column for date followed by another column for work done. It is composed of five days from Monday to Friday. At the bottom of the page is a space for signature by the Industrial Based (I.B) Supervisor.

Since the IT is long and the daily activities of pharmacy IT students is few and almost repetitive, it is best to spread the work done over a long period. Knowledge on how to change writing style I very important here. Since the logbook will not just sit at the faculty of pharmacy but will go to IT office, better be prepared to impress not just your pharmacy lecturers but also those strangers out there.

The first day should contain something like an introduction to the establishment. This can be followed the next day with an introduction to the pharmacy. There are definitely different units in the establishment, which may need some touches. For instance, a hospital pharmacy may have up to 4 units although depends on how large it is. Also, a community pharmacy can be divided. However, in a situation whereby your IT venue has just one or few units, you can as well skip this part. Generally, the first week should be an introduction. At the end of the week, the summary with a diagram of the establishment with a brief history will do.


The following weeks should contain various activities. Never pack all the activities you did in a day in the days work. You will find it hard to fill up the rest days. Include activities you are supposed to do yet could not do it due to factors beyond your control but you know pharmacy do.

Always remember to include images with a pencil. Images may range from syringes, needles, etc.
At the end of everything, your IB Supervisor will grade your activities in the period under review from dressing to hard work and others.

There are other forms not found in the logbook but need to be filled. These include the Industrial training assessment Sheet. Others are End Of Program Evaluation contain space for name, department, establishment, performance rating filled by IB Supervisor and duration of IT.
